(a) In order to support the reports required of participants, the sponsor of group or self-study programs shall retain the following records:
- (1) record of participation, e.g., sign-in sheet reflecting the credit hours earned by each participant including those who arrive late or leave early;
- (2) course program including recommended credit hours as required by §523.140(b) of this chapter (relating to Program Standards);
(3) documentation as required by §523.140 of this chapter and:
- (A) all promotional materials;
- (B) date(s); and
- (C) location.
- (4) instructor(s), including resume or biography;
- (5) evaluation of program as directed in §523.141(b) of this chapter (relating to Evaluation);
- (6) pre-test data for self-study courses; and
- (7) all final exams completed by participants for self-study courses.
- (b) Documentation shall be retained for five years from the date the program is completed.
