How- $ 25 ever, if the applicant is a doctor of veterinary medicine seeking provisional licensure, a true $ 25 and accurate copy of the applicant’s diploma $ 50 or a certified letter from the dean of the accredited school or college of veterinary $ 25 medicine from which the applicant graduated will be acceptable proof of educational credentials of said applicant for provisional licensure only;
photos, black and white or color, one and one-half inches by two inches (1.5" × 2.0"), with applicant’s signature on the back of each.
