1. (1) Application for licensure or registration

$ 50 typed), signed, notarized and accompanied $ 50 by the appropriate fees. The fee must be in the form of a cashier’s check, personal check $ 50 or money order. $ 20

1. (3) The following documents must be on file

$ 15 dentials as evidenced by an official transcript sent directly to the board by the school. How- $100 ever, if the applicant is a doctor of veterinary $100 medicine seeking provisional licensure, a true and accurate copy of the applicant’s diploma $100 or a certified letter from the dean of the $ 25 accredited school or college of veterinary $ 25 medicine from which the applicant graduated will be acceptable proof of educational cre- $ 25 dentials of said applicant for provisional $ 50 licensure only;

1. (D) Two (2) current, standard passport

$ 25 photos, black and white or color, one and one-half inches by two inches (1.5" × 2.0"), with applicant’s signature on the back of each.
