1. (1) Application for licensure or registration must be made on the forms provided by the

$ 15 board. Application forms may be obtained by requesting them from the executive director, $ 50 Missouri Veterinary Medical Board, PO Box $ 30 633, Jefferson City, MO 65102. $200 $ 50

1. (2) An application must be legible (printed or

$ 50 typed), signed, notarized and accompanied $ 50 by the appropriate fees. The fee must be in the form of a cashier’s check, personal check $ 20 or money order. $ 10 $ 50

1. (3) The following documents must be on file

$ 15 for an application to be considered complete:

1. (A) Completed application;

2. (C) Proof of acceptable educational cre-

$100 dentials as evidenced by an official transcript $100 sent directly to the board by the school. However, if the applicant is a doctor of veterinary $100 medicine seeking provisional licensure, a true $ 25 and accurate copy of the applicant’s diploma $ 25 or a certified letter from the dean of the accredited school or college of veterinary $ 25 medicine from which the applicant graduated $ 50 will be acceptable proof of educational credentials of said applicant for provisional $ 25 licensure only;

1. (D) Two (2) current, standard passport photos, black and white or color, one and one-half inches by two inches (1.5" × 2.0"), with applicant’s signature on the back of each.
