CHAPTER 2
APPLICATION PROCEDURES
Section 1. License or Registration Types. The Board is autho- rized to issue licenses or registrations to those applicants who meet the educational, experience, examination and other qualifications required by the Act and these rules.
These include:
- (a) Physical Therapist license;
- (b) Temporary Physical Therapist license;
- (c) Temporary Foreign-Trained Physical Therapist license;
- (d) Physical Therapist Assistant registration;
- (e) Temporary Physical Therapist Assistant registration;
Section 2. Application Requirements. Every applicant for a license to practice as a physical therapist or registration as a physical therapist assistant shall submit the following materials:
- (a) A completed and notarized application form as provided by the Board;
- (b) All appropriate fees paid only by cashier's check or money order;
- (c) A passport-size colored photograph;
- (d) Official transcripts from physical therapy school(s) at- tended mailed directly to the Board by the respective institution; the transcript must indicate when the physical therapy or physical therapy assistant degree was awarded;
- (e) A copy of the national examination scores recommended by the Federation of State Boards of Physical Therapy mailed directly to the Board by the source;
- (f) A verification letter from the three (3) most current state licensing boards within which the applicant is or was licensed or registered to practice physical therapy or physical therapist assis- tant duties;
- (g) Two (2) professional references mailed directly to the Board by the reference including the reference's complete name, address, telephone number and original signature;
- (h) Credential report for foreign-educated applicants mailed directly to the Board by one of the following credentials evaluation services: International Credentialing Associates, International Con- sultants of Delaware, Inc., or International Education Research Foun- dation, Inc;
- (i) Supervision form for temporary license applicants mailed directly to the Board by the supervisor;
- (j) The Board will accept electronic information including school transcripts, PES scores, credential reports for foreign trained applicants, and disciplinary actions from the Federation of State Boards National Records Database, and other state boards.
Section 3. When Eligibility is Unclear. When the Board is un- able to ascertain the qualifications of the applicant it may request the applicant to provide additional documentation or information. The Board may also request the applicant to appear before the Board to clarify issues relating to qualifications.
Section 4. Issuance of License or Registration. Upon verifica- tion and acceptance by the Board of the applicant's documentation of qualifications, the applicant shall be granted licensure or registra- tion and the Board shall issue to each successful candidate a wall certificate bearing the date of issuance and the seal of the Board.
The Board shall also enclose a pocket card bearing the expiration date and a copy of the rules and regulations which indicate the terms of the license or registration.
