CHAPTER 2
LICENSING REQUIREMENTS AND PROCEDURES
Section 1. General Licensure Requirements and Procedures.
- (a) Requirements. All applications for licensure shall include:
- (i) A completed, notarized Board application form with a recent color photograph of the applicant;
- (ii) The non-refundable application fee as set by the Board;
- (iii) An official copy of the National Physical Therapist Examination (NPTE) sent by the authorized score reporting service;
- (iv) License verification(s) from states in which the applicant is or was licensed or registered to practice physical therapy or physical therapist assistant duties;
- (v) Two (2) professional references mailed directly to the Board including the reference's complete name, title, address, telephone number and original signature;
- (vi) Proof of Lawful Presence in the United States;
- (vii) Documentation of academic qualifications;
- (A) For applicants who completed their physical therapy education in the U.S., the documentation required is:
- (I) An official transcript showing completion of an accredited physical therapy or physical therapist assistant program approved by the American Physical Therapy Association (APTA).
- (B) For applicants who completed their physical therapy education outside the U.S., the documentation required is:
- (I) The Board will establish by policy a list of approved credentialing entities, which will be made available to the foreign-trained applicant on request.
- (II) The credentialing entity must attest that the institution attended by the applicant is substantially equivalent to similar institutions located in the United States and has the recognition of the Ministry of Education or the equivalent in that country. The coursework content should
be substantially equivalent to coursework and clinical preparation completed by graduates of accredited programs in the United States.
(III) If the credential evaluation is accepted by the Board, the applicant will be considered for a temporary license.
(b) An applicant may take the examination for PT or PTA licensure only after the application process is complete and all requirements are met.
(c) Application expiration. An application for licensure is valid for one year after the date it is received by the Board.
(d) False information. An applicant who submits an application containing false information may be denied licensure by the Board.
Section 2. Eligibility. When the Board is unable to ascertain the qualifications of the applicant it may request the applicant provide additional documentation or information. The Board may also request the applicant appear before the Board to clarify issues relating to qualifications.
Section 3. Issuance of License or Registration. Upon verification and acceptance by the Board of the applicant's documentation of qualifications, the applicant shall be granted licensure or registration. The Board shall issue to each successful candidate a wall certificate bearing the date of issuance and the seal of the Board. The Board shall also include a pocket card bearing the expiration date of the license or registration.
