| (h) | Salon/School late fee(up to 30 days) | $ 10.00 |
| (i) | Salon/School late fee(up to 60 days) | $ 20.00 |
| (j) | Salon/School late fee(up to 90 days) | $ 30.00 |
| (k) | School application fee | $300.00 |
| (l) | School license (renewal) | $150.00 |
| (m) | Endorsement fee | $100.00 |
