(ii) Complete and notarized application for renewal;
(iii) Verification of compliance with the continuing education requirements as set out below.
(b) Each Licensee shall earn a minimum of forty-five (45) clock hours of continuing education every two (2) years.
(i) Clock hours for purposes of this section shall be the actual number of hours (contact hours) during which instruction was received. For academic courses, one (1) semester credit equals fifteen (15) contact hours. One (1) CEU credit is equal to ten (10) clock hours.
(ii) Only those hours acquired during the current two (2) year renewal period will be considered.
(A) For initial renewal, the two (2) year renewal period begins on the license issue date and ends on the expiration date as reflected on documents provided by the Board.
(B) For subsequent renewals, the two (2) year renewal period begins on the last date renewed and ends on the current expiration date as reflected on documents provided by the Board.
(iii) The following standards shall govern acceptability of continuing education activities:
(A) They shall have significant intellectual or practical content, and the primary objective shall be to increase the participant's competence within each licensed discipline.
(B) They shall constitute an organized program of learning dealing with matters directly related to the clinical practice, professional responsibility or ethical obligations of each discipline.
(C) Presenters of activity should be experts in the discipline and of at least master's degree level in education. The scope of practice for each discipline in these rules and regulations may be used as a basis of what knowledge and skills are acceptable to the Board as continuing education activities.
(iv) Each Licensee shall submit an affidavit on a form approved by the Board attesting to the number of continuing education hours completed. The affidavit shall state that the applicant is familiar with the requirements for continuing education credit and has attended the appropriate workshop or training sessions.
(v) With the affidavit, the applicant must submit a listing of the continuing education activity on the form provided by the Board.
(A) In addition to the listing, the Licensee must attach copies of any certificates of attendance, letters certifying attendance, transcripts, or any official documents which serve as proof of participation or attendance.
(B) Proof of attendance must contain the workshop title, dates, clock hours attended, sponsor and sponsor's signature, presenter, qualifications of the presenter and the name of the Licensee.
(vi) Licensees are responsible for maintaining their own CE documentation files. Sixty (60) days prior to the license renewal date the Board will send notice to the Licensee at their last address of record to submit their renewal fee and documentation to the Board for review.
(vii) License renewal documents will not be accepted more than one hundred twenty (120) days prior to the license expiration date.
(viii) License renewal documents received which are postmarked after the license expiration date will not be accepted by the Board and will be returned to the Licensee and the license will become null and void. Any Licensee who allows their license to lapse but wishes to maintain a Wyoming license must comply with the requirements for re-licensure as set out in Chapter III, Section 7.
(ix) Failure to receive notice and application for renewal of license from the Board does not excuse a Licensee from the requirement for renewal under the Act and these rules.
