(a) Initial licenses issued by the Board shall be valid for a period of two (2) years. A separate and distinct license shall be issued for each of the four (4) disciplines involved. These licenses are:
(b) An applicant shall clearly indicate the specific license desired. An applicant who seeks licensure in more than one discipline shall submit separate applications, appropriate fees and documentation for each license.
(c) Any subsequent license in another discipline issued to the same person shall expire on the same date as the first license held.
Section 2. Application by Examination Procedures. The completed application must be received and accepted by the Board at least four (4) months prior to the scheduled administration of the examination for the appropriate discipline or the applicant will be required to wait until the next administration of the exam. The following documents are required for a complete application:

(e) All documents submitted in support of the application must contain an original signature and be submitted directly to the offices of the Board from the respondent and not forwarded through the applicant.
(f) The Board will not accept faxed or photocopied documents.
Section 3. When Eligibility is Unclear. If the Board is unable to ascertain from documents submitted by the applicant that the applicant's training makes them eligible to take the examination, the Board may require the applicant to provide additional documentation or information deemed by the Board as necessary to make that decision. Oral interviews may be requested.
Section 4. Application by Persons Licensed in Other Jurisdictions Procedures. The following are required for a complete application.
(a) A complete and notarized official application form accompanied by the application fee.
(b) Official transcripts of graduate course work from all colleges and universities attended.
(c) Verification of licensure in good standing from another jurisdiction. Applicants licensed for less than two (2) years shall provide verification of supervised experience as required in these rules.
(d) References from three (3) professionals familiar with the applicant which address the applicant's abilities and professional performance in the discipline for which the license is requested;
(i) References must have been written within six (6) months of the date of application.
(ii) References will not be accepted from relatives of the applicant.
(e) The Board may deny licensure by endorsement and require examination if the qualifications are unclear or unacceptable.
(f) All documents submitted in support of the application must contain an original signature and be submitted directly to the offices of the Board from the respondent and not forwarded through the applicant.
(g) The Board will not accept faxed or photocopied documents.
Section 5. Notification of Applicants and Right of Appeal. If the Board approves the applicant's initial application, this finding will be communicated in writing to the applicant along with information and instruction for applying and sitting for the written examination. If the applicant is denied on the basis of initial application materials, the reasons for this rejection shall be communicated in writing. The applicant shall have the right of reconsideration based on submission of new information and/or an appearance before the Board with the opportunity to demonstrate to the Board that they meet the licensure requirements.
Section 6. Issuance of License. The Board shall issue to the successful applicant a license bearing the full name of the license holder, discipline designation, date of issuance, expiration date, a serial number and appropriate seal.
(a) Licensees must register with the Board any change in their legal name within thirty (30) days of the change, must submit documentation demonstrating the change of name, appropriate fee and must surrender the old certificate and a new certificate shall be issued by the Board.
(b) Each applicant and Licensee must file with the Board their current home and professional mailing addresses and telephone numbers and, within thirty (30) days of the change, must report to the Board in writing any change of addresses or telephone numbers, giving both old and new addresses and telephone numbers.
(a) Release of records shall be governed by W.S. 16-4-201 et seq., Public Records Act.
(b) Any applicant, Licensee or others with proper notarized written consent may personally inspect the contents of a Board file with the exception of personal recommendations or verification of supervised experience.
(c) Record inspection shall take place under the following conditions:
(i) An appointment must be made to review the file between the hours of 8:00 a.m. and 5:00 p.m., Monday through Friday at the offices of the Board.
(ii) Record inspection must take place in the presence of a member of the Board or a representative of its administrative staff.
(iii) Original documents must remain with the Board but may be copied at the Board offices for a reasonable fee.
Section 9. Correction and Amendment. Any applicant or Licensee may clarify erroneous, inaccurate or misleading information in their file by submitting a written statement to the Board which will be placed in their file.
