Fees

“Emergency rules are in effect no longer than 120 days after filing with the Registrar of Rules”

Section 1. Statement of Purpose. These Board Rules are adopted to implement the Board's authority to determine and collect reasonable fees.

Section 2. General Information.

1. (a) Fees shall be payable in the exact amount by money order, cashier's check, or certified check. Application fees and renewals may be paid by personal check.
2. (b) All fees collected by the Board are non-refundable.

Section 3. Fees. Services for which the Board charges a fee shall include but is not limited to the following fee schedule.

2. (i) Provisional $200
3. (ii) Initial License by Completing Provisional $100
4. (iii) Initial License $300
5. (iv) Endorsement $300
6. (b) Annual License Renewal Fee $100
7. (c) Relicensure Application Fee $300
8. (d) Reinstatement Application Fee $350
9. (e) Certification as an SLPA
10. (i) Initial $100
11. (ii) Annual Renewal $50
12. (g) Aide or Audiometric Technician Registration Fee $25
13. (h) Roster $100

(i) Duplicate Pocket Card or Wall Certificate $25 (j) License or Certificate Verification $25 (k) Non-sufficient Funds $30 (l) Copy Charge 20¢/page
