Chapter 7

Fees

Section 1. Authority and Statement of Purpose. These Board Rules are adopted to implement the Board's authority to determine and collect reasonable fees granted by Wyoming Statute 33-33-307.

Section 2. General Information.

1. (a) Fees shall be payable in the exact amount and shall be paid in advance of the services rendered.
2. (b) All fees collected by the Board are non-refundable.
3. (c) Fees related to public records requests shall be assessed pursuant to the Department of Administration and Information rules concerning public records.

Section 3. Fees. Services for which the Board charges a fee shall include but is not limited to the following fee schedule.

1. (a) Application Fee
2. (i) Provisional $200
3. (ii) Initial License by Completing Provisional $100
4. (iii) Initial License $300
5. (iv) Endorsement $300
6. (v) Compact Home State $300
7. (b) Annual License Renewal Fee $100
8. (c) Compact Privilege to Practice Fee $120
9. (d) Relicensure Application Fee $300
10. (e) Reinstatement Application Fee $350
11. (f) Certification as an SLPA
12. (i) Initial $100
13. (ii) Annual Renewal $50

(ii) Certificate Re-issuance Fee $100 (g) Aide or Audiometric Technician Registration Fee $25 (h) Duplicate Pocket Card or Wall Certificate $25 (i) License or Certificate Verification $25 (j) Non-sufficient funds fee will be charged in accordance with W.S. 1-1-115
