(a) The board hereby establishes non-refundable fees in the amounts indicated, for the following:

| (iii) | Duplicate Certificate | 5.00 |
| (iv) | Temporary Permit to Practice | 60.00 |
| (v) | Restoration fee after one year—Barber | 75.00 |
| (vi) | Restoration fee after two years—Barber | 150.00 |
| (vii) | Shop license (annual) | 60.00 |
| (viii) | Shop renewal fee after July 31 of each year | 65.00 |
| (ix) | New shop or school license inspection | 100.00 |
| (x) | Change of Location of Inspection | 100.00 |
| (xi) | Reopening inspection | 100.00 |
| (xii) | Barber school application | 150.00 |
| (xiii) | Barber school license (annual) | 100.00 |
| (xiv) | Instructor examination | 100.00 |
