A monthly disability benefit is not payable for a total disability proximately caused by:

1. (1) self-inflicted injury;
2. (2) abuse or misuse of any substance, including alcohol;
3. (3) illicit drug use;
4. (4) an act of aggression committed by the eligible employee; or
5. (5) the eligible employee's commission or attempted commission of a felony or other illegal act.
