Upon request, the commissioner shall furnish approved forms of the application for office relocation or application for change of name. The Savings and Loan Department hereby adopts by reference the application for home office relocation, application for branch office relocation, and application for change of name. Copies of the applications may be obtained from the Texas Savings and Loan Department, 2601 North Lamar Boulevard, Suite 201, Austin, Texas 78705.
