Upon request, the commissioner shall furnish approved forms of the application for office relocation or application for change of name. Copies of the applications may be obtained from the Texas Department of Savings and Mortgage Lending, 2601 North Lamar Boulevard, Suite 201, Austin, Texas 78705.
