1. (a) When a licensee is arrested, charged, or indicted for a criminal offense above the grade of Class C misdemeanor or for any Class C misdemeanor involving the duties and responsibilities of office or family violence, that person must report such fact to the commission in writing within 30 days, including the name of the arresting agency, the style, court, and cause number of the charge or indictment, if any, and the address to which notice of any commission action will be mailed.
2. (b) A person to whom this section applies must also report to the commission the final disposition of the criminal action within 30 days of the effective date of the disposition.
3. (c) A licensee must report any name change to the commission within 30 days.
4. (d) A licensee must report to the commission a permanent mailing address other than an agency address and must report to the commission any change within 30 days.
5. (e) A licensee must report all subsequent DD214's to the Commission indicating any military discharge other than under honorable or general-under-honorable conditions within 30 days.
6. (f) The effective date of this section is March 1, 2008.
