Documentation of each service contact must include:

1. (1) the name of the child;
2. (2) the name of the ECI contractor and the name and the discipline of the service provider;
3. (3) the date, start time, length of time, and place of service;
4. (4) method (individual or group);

5. (5) a description of the techniques by which the provider engaged the family or routine caregiver in activities to meet the developmental needs of the child. This includes:

   1. (A) coaching and instructions to the family or caregiver;
   2. (B) discussing how activities apply to child and family routines; and
   3. (C) modeling intervention techniques within everyday learning opportunities, including a description of the opportunity for the caregiver's return demonstration;
6. (6) the IFSP goal that was the focus of the intervention;
7. (7) the child's progress related to the goals in the IFSP;
8. (8) relevant new information about the child provided by the family or other routine caregiver; and
9. (9) the service provider's signature.
