1. (a) All programs should include some means for evaluating quality by both participants and instructors to determine whether:

   1. (1) objectives have been met;
   2. (2) prerequisites were necessary or desirable;
   3. (3) facilities were satisfactory;
   4. (4) the instructor was effective;
   5. (5) advance preparation materials, if any, were satisfactory; and
   6. (6) the program content was timely and effective.
2. (b) Evaluations should consist of evaluation forms or questionnaires upon completion of the program.
3. (c) Instructors should be informed of their performance, and sponsors should systematically review the evaluation process to ensure its effectiveness.
4. (d) Sponsors are responsible for collecting evaluation forms from CPA participants.
