1. (a) A person whose certificate has been revoked because of a criminal conviction may request a reinstatement when the following terms have been met:

   1. (1) successful completion of all obligations to the criminal justice system; and
   2. (2) the person shall report that they have had no criminal convictions, within a two-year period, beginning from the date the terms set forth in paragraph (1) of this subsection were satisfied.
2. (b) All requests for reinstatement must be in writing to the board.
3. (c) The request for reinstatement shall include proof that the terms set forth in subsection (a)(1) of this section have been successfully completed, with the request for reinstatement.
4. (d) Reinstatement will be at the sole discretion of the board.
