(a) A person whose certificate has been revoked because of a criminal conviction may request a reinstatement when the following terms have been met:
- (1) successful completion of all obligations to the criminal justice system; and
- (2) the person shall report that they have had no criminal convictions, within a two-year period, beginning from the date the terms set forth in paragraph (1) of this subsection were satisfied.
- (b) All requests for reinstatement must be in writing to the board.
- (c) The request for reinstatement shall include proof that the terms set forth in subsection (a)(1) of this section have been successfully completed, with the request for reinstatement.
- (d) Reinstatement will be at the sole discretion of the board.
