The Board shall transfer a license to a person who:

1. (1) meets the requirements set forth in the Plumbing License Law; and

2. (2) submits the following:

   1. (A) the required application;
   2. (B) the required fee; and
   3. (C) a notarized Transfer of License Affidavit that is signed by the transferor or a valid will executed by the transferor.
