1. (a) All fees paid to the department are nonrefundable.

2. (b) Driving Safety School Fees:

   1. (1) The initial application fee for a driving safety school is $150.
   2. (2) The fee for a change of address of a driving safety school is $50.
   3. (3) The fee for a change of name of a driving safety school or name of owner is $50.
   4. (4) If a driving safety school changes ownership as defined under §84.2(6), the fee paid by the new owner is $150.

3. (c) Driving Safety Instructor Fees:

   1. (1) The initial application fee for a driving safety instructor license is $50.
   2. (2) The initial application fee for a specialized driving safety instructor license is $50.
   3. (3) The annual renewal application fee for a driving safety instructor license is $25.
   4. (4) The annual renewal application fee for a specialized driving safety instructor license is $25.

4. (d) Driving Safety Course Provider Fees:

   1. (1) The initial application fee for a course provider is $2,000.
   2. (2) The annual renewal application fee for a course provider is $200.
   3. (3) The fee for a change of address of a course provider is $50.
   4. (4) The fee for a change of name of a course provider or name of owner is $100.
   5. (5) If a driving safety course provider changes ownership as defined under §84.2(6), the fee paid by the new owner is $2,000.

5. (e) Driving Safety Course Fees:

   1. (1) The fee for a driving safety course approval is $9,000.
   2. (2) The fee for a specialized driving safety course approval is $9,000.
   3. (3) The application fee for each additional course for a driving safety school is $25.
   4. (4) The fee for a course completion certificate number is $1.00.

6. (f) Other Fees:

   1. (1) A duplicate/replacement fee for any license issued under this chapter is $25.
   2. (2) Late renewal fees for licenses issued under this chapter are provided under §60.83 of this title (relating to Late Renewal Fees).
   3. (3) A dishonored/returned check or payment fee is the fee prescribed under §60.82 of this title (relating to Dishonored Payment Device).
   4. (4) The fee for a criminal history evaluation letter is the fee prescribed under §60.42 of this title (relating to Criminal History Evaluation Letters).
