1. (a) All fees paid to the department are nonrefundable.

2. (b) Driving Safety Provider Fees:

   1. (1) The initial application fee for a driving safety provider is $500.
   2. (2) The annual renewal application fee for a driving safety provider is $100.
   3. (3) The fee for a change of address of a driving safety provider is $50.
   4. (4) The fee for a change of name of a driving safety provider or name of owner is $50.
   5. (5) If a driving safety provider changes ownership as defined under §84.60(e), the fee paid by the new owner is $500.
3. (c) Driving Safety Course Completion Certificate Fees: The fee for a course completion certificate number is $1.00.

4. (d) Other Fees:

   1. (1) A duplicate/replacement fee for any license issued under this chapter is $25.
   2. (2) Late renewal fees for licenses issued under this chapter are provided under §60.83 of this title (relating to Late Renewal Fees).
   3. (3) A dishonored/returned check or payment fee is the fee prescribed under §60.82 of this title (relating to Dishonored Payment Device).
   4. (4) The fee for a criminal history evaluation letter is the fee prescribed under §60.42 of this title (relating to Criminal History Evaluation Letters).
