1. (a) Provider registrations are valid for one year and may be renewed at the end of each registration period.

2. (b) To renew a registration, a provider shall:

   1. (1) file a completed application for renewal on the appropriate department-approved form; and
   2. (2) pay all applicable fees.
3. (c) Late renewal fees for registrations issued under this chapter are provided under §60.83 of this title (relating to Late Renewal Fees).
