1. (a) Provider registrations are valid for one year and may be renewed at the end of each registration period.

2. (b) To renew a registration, a provider shall:

   1. (1) file a completed application for renewal on the appropriate department-approved form;
   2. (2) pay all applicable fees; and
   3. (3) be current on the payment of any unpaid required fees, including record fees, or administrative penalties.
3. (c) Late renewal fees for registrations issued under this chapter are provided under §60.83 of this title (relating to Late Renewal Fees).
