   2. (2) Extended Temporary License extension--$50
   3. (3) Provisional License--$125
   4. (4) Doctor of Podiatric Medicine Initial License--$750
   5. (5) Doctor of Podiatric Medicine Renewal License--$700
   6. (6) Voluntary Charity Care Status License (Initial and Renewal)--$0
   7. (7) Active Duty Military Members--$0
   8. (8) Hyperbaric Oxygen Certificate--$25
   9. (9) Nitrous Oxide Registration--$25
   10. (10) Podiatric Medical Radiological Technicians--$25
   11. (11) Duplicate License/replacement license--$25
   12. (12) The fee for a criminal history evaluation letter is the fee prescribed under §60.42 (relating to Criminal History Evaluation Letters).
   13. (13) A dishonored/returned check or payment fee is the fee prescribed under §60.82 (relating to Dishonored Payment Device).
   14. (14) Late renewal fees for licenses issued under this chapter are provided under §60.83 (relating to Late Renewal Fees).
