   2. (2) Extended Temporary License extension--$50
   3. (3) Provisional License--$125
   4. (4) Doctor of Podiatric Medicine Initial License--$750
   5. (5) Doctor of Podiatric Medicine Renewal License--$700
   6. (6) Voluntary Charity Care Status License (Initial and Renewal)--$0
   7. (7) Hyperbaric Oxygen Certificate--$25
   8. (8) Nitrous Oxide Registration--$25
   9. (9) Podiatric Medical Radiological Technicians--$0
   10. (10) Duplicate License/replacement license--$25
   11. (11) The fee for a criminal history evaluation letter is the fee prescribed under §60.42 of this title (relating to Criminal History Evaluation Letters).
   12. (12) A dishonored/returned check or payment fee is the fee prescribed under §60.82 of this title (relating to Dishonored Payment Device).
   13. (13) Late renewal fees for licenses issued under this chapter are provided under §60.83 of this title (relating to Late Renewal Fees).
