1. (a) Applications for licenses, permits and certificates shall be made by an applicant in such a manner as may be directed by the administrator upon forms provided by the commission.
2. (b) Each application shall include all information required by the administrator to insure compliance with all applicable statutes and rules and regulations of the agency.
3. (c) Each applicant for a license, permit or certificate issued by the commission, shall submit with the application the payment of all state fees and surcharges in accordance with the applicable provisions of the code and rules.
