1. (a) Applications for licenses, permits and certificates shall be made by an applicant in such a manner as may be directed by the executive director upon forms provided by the commission.
2. (b) Each application shall include all information required by the executive director to ensure compliance with all applicable statutes and rules and regulations of the agency.
3. (c) Each applicant for a license, permit or certificate issued by the commission, shall submit with the application the payment of all required fees in accordance with the applicable provisions of the code and rules.
4. (d) License and permit renewals which are required to be submitted directly to the licensing division in Austin must be postmarked no later than thirty calendar days after expiration or received by the commission no later than thirty calendar days after expiration. The application must be complete in form and accompanied by all state fees, surcharges, and late fees.
