1. (1) When unpaid installments are deferred as provided by ORS 725.340(2)(b), the licensee must give the borrower written evidence of the agreed deferral showing:

   1. (a) The amount of the deferral charge;
   2. (b) The new due date of the first deferred installment; and
   3. (c) The new due date of the final deferred installment of the loan.
2. (2) The licensee must also note the due date of the final deferred installment and the amount of the deferral charge on the borrower's account record.
