PURPOSE: This rule establishes the fees for the Division of Professional Registration and the Missouri Board for Respiratory Care.
(1) The following fees are established by the Division of Professional Registration:
- (A) Application for Licensure as a Respiratory Care Practitioner (Including Reciprocity Applications) $ 40.00
- (B) Application for Temporary Permit $ 20.00
- (C) Application for an Educational Permit $ 15.00
- (D) Endorsement to Another Jurisdiction $ 15.00
- (E) Duplicate License/Permit Fee $ 10.00
- (F) Replacement Wall-Hanging (Ornamental Certificate) $ 15.00
- (G) Insufficient Funds Check Fee Charge $ 25.00
- (H) Biennial License Renewal Fee $ 30.00
- (I) Late Renewal Penalty Fee $100.00
- (J) Biennial Inactive License Renewal Fee $ 20.00
- (2) All fees are nonrefundable.
