PURPOSE: This rule establishes application and course approval fees.

1. (1) The following fees are established for school accreditation:

   1. (A) Initial School Accreditation Application Fee $200

   2. (B) Course Approval Application Fee for courses over twelve

      1. (12) hours (per course per delivery method) $400 AND INSURANCE
2. (C) Course Approval Application Fee for courses less than or equal to twelve (12) hours (per course per delivery method)
3. (D) School Accreditation Renewal Application Fee

4. (2) The following fees have been established for course approval:

   1. (A) Course Renewal Application Fee for course over twelve

      1. (12) hours
   2. (B) Course Renewal Application Fee for course less than or equal to twelve (12) hours (per course per delivery method)
