PURPOSE: This rule provides for evaluation of the professional nursing program by students and faculty.

1. (1) There shall be a written plan for systematic evaluation of all aspects of the program. The plan shall include:

   1. (A) Frequency of evaluation;
   2. (B) Methods of evaluation; and
   3. (C) Person(s) responsible for the evaluation.

2. (2) The systematic evaluation plan provides for the evaluation of the following:

   1. (A) Clinical sites by students and faculty;
   2. (B) Course and faculty by students;
   3. (C) Students and faculty by representatives of clinical site(s); and
   4. (D) Program preparation for nursing employment by graduate nurses, six (6) months or more, after graduation.
3. (3) Documentation shall indicate that the systematic evaluation plan has been utilized in the planning and improvement of the program.
