$ 20 PURPOSE: This rule requires licensees to inform the Missouri Dental Board of any $ 50 name or address changes. $ 5 Licensees shall keep the board informed of their legal name, mailing address and telephone number. Any change shall be reported to the board in writing within thirty (30) $ 25 days.
