$ 10 $ 10 PURPOSE: This rule requires licensees to inform the Missouri $ 10 Dental Board of any name or address changes. $100 $100 Licensees shall keep the board informed of their legal name, mailing address and telephone number. Any change shall be $ 15 reported to the board in writing within thirty (30) days.
