On completion of the drilling of any well, the well driller shall file a final report with the Department. The report shall include:

1. (1) The log of each well;
2. (2) The size and depth of each well;
3. (3) The diameters and lengths of casing and screen installed;
4. (4) The static and pumping levels;
5. (5) The yield of each well; and
6. (6) Any other information relating to the construction or operation of the well that the Department requires.
