1. (a) A statement that the information is unknown to such person (and, if a corporation or partnership, to all officers, directors and partners thereof);
2. (b) A statement of all efforts made to obtain such information and the reasons ascribed to their failure; and
3. (c) A statement in reasonable detail as to the reasons why further efforts to obtain such information should be deemed to involve unreasonable effort or expense.

Failure to supply information with the application which is sought to be excused under the provisions of subdivision (d) of Section 25131 of the Code shall be supported by an affidavit. The affidavit shall be executed by the applicant and by each person on whose behalf the distribution is to be made and shall include, as to each deficiency:
