COSTS

Section 1. Fees. This fee schedule is adopted by the Board pursuant to W.S. 33-1-201.

(a) Application Fees:

(i) Licensure $200.00 per discipline

(ii) Certification $150.00 per discipline

(iii) Provisional License $100.00 per discipline

(b) License and Certification Fees: $100.00 per discipline

(c) Renewal Fees:

(i) License $100.00 for the initial discipline and $50.00 for every additional discipline.

(ii) Certification $75.00 per discipline

(d) Examination Fees: Actual cost of test and administration

(e) Non-sufficient Funds Fee: $15.00

(f) License and Certification Verification Fee: $50.00

(g) Replacement Rules and Regulations $10.00

Section 2. Refunds.

(a) Fees shall be paid in the exact amount, by money order or cashier's check, and shall be paid in advance of services rendered. Fees for renewal of license or certification may be paid by personal or business check.

(b) All fees collected by the Board are non-refundable.

Section 3. Applications Unaccompanied by Fees. No application shall be considered by the Board unless accompanied by the application fee.

Section 4. Forfeiture of Examination Fees. If the applicant fails to appear for a scheduled examination, the examination fee shall be forfeited.

Section 5. Duplicate or Replacement Certificate and Pocket Card. Duplicate or replacement wall certificates and pocket cards may be issued by the Board. All requests for duplicate or replacement certificates must be in writing and must be accompanied by a $10.00 fee for each. All requests for duplicate or replacement pocket cards must be in writing and must be accompanied by a $5.00 fee for each.

Section 6. Requests for Roster of Licensees and Certificates. The roster of current Licensees and Certificates shall be updated semi-annually and made available upon written request and payment of required publication fee. Any request for the roster must be accompanied by a complete Release of List of Licensees and Certificates form and payment as follows:

1. (a) The roster shall be made available to governmental agencies, academic institutions and appropriate professional associations upon request.
2. (b) Any other organization may obtain a roster by submitting a money order or cashier's check in the amount of $100.00 accompanied by a written request.
3. (c) Licensees and Certificates will receive a complimentary copy of the current roster upon issuance of a license or certification. Subsequent rosters may be obtained for a fee of $25.00.
