Section 1. Authority. This fee schedule is promulgated by the Board pursuant to Wyoming Statute 33-1- 201.
(a) Fees shall be payable in the exact amount for all services and shall be paid in advance of the services rendered.
(b) All fees collected by the Board are non-refundable.
(a) Application Fees:
(i) Licensure by reciprocity $375.00 per discipline
(ii) Licensure by examination $375.00 per discipline
(iii) Licensure by completing provisional $200.00 per discipline
(iv) Certification by reciprocity $325.00 per discipline
(v) Certification by examination $325.00 per discipline
(vi) Provisional License $175.00 per discipline
(vii) Compact Home State $375.00
(b) Renewal Fees:
(i) License $200.00 for the initial discipline and $75.00 for every additional discipline.
(ii) Certification $125.00 per discipline
(c) Re-Licensure Fee $375.00 per discipline
(d) Re-Certification Fee $325.00 per discipline
(e) Re-Instatement of license or certificate $200.00 per discipline
(f) Compact Privilege to Practice Fee $250.00 (g) Non-sufficient Funds Fee: in accordance with W.S. 1-1-115 (h) Verification Fee: $30.00. (i) Background Check Fee $39.00 - required with initial application and renewal every 4 years. (j) National Practitioners Data Base (NPDB) Fee $5.00 - required with initial application and each renewal. (k) Duplicate Pocket Card $10.00 (l) Duplicate Wall Certificate $20.00
Section 4. Applications Unaccompanied by Fees. No application shall be considered by the Board unless accompanied by all fees.
Section 5. Requests for Roster of Licensees and Certificate Holders. The roster of current licensees and certificate holders shall be updated at least annually and made available electronically at no charge.
