CHAPTER 3

APPLICATION PROCESS

Section 1. Application for Licensure. All applicants for licensure must submit the following documents to the Board at least thirty (30) days prior to the next scheduled examination:

1. (a) A completed and legible application with a recent unmounted full-face photograph.
2. (b) Required fee in the form of a money order or cashier's check.
3. (c) An official transcript which reflects the award of a degree in optometry, mailed directly to the Board.
4. (d) Passing score verification of the Parts one (1), two (2), and three (3), and the treatment and management of ocular disease (TMOD) section of the examination from the National Board of Examiners in Optometry mailed directly to the Board.
5. (e) One (1) letter of professional recommendation which attests to the applicant's good reputation for honesty, trustworthiness, integrity and competence in the practice of optometry, from a reputable practitioner of optometry, mailed directly to the Board.
6. (f) Individuals requesting special accommodations due to a disability must notify the Board in writing in advance of the examination so that appropriate arrangements can be made.

Section 2. Notification of Applicants and Right of Appeal. If the Board approves the applicant's initial application, this finding will be communicated in writing to the applicant along with information and instruction for sitting for the State examination. If the applicant is denied on the basis of initial application materials, the reasons for this rejection shall be communicated in writing. The applicant shall have the right of reconsideration based on submission of new information and/or an appearance before the Board with the opportunity to demonstrate to the Board that the licensure requirements are met.
