CHAPTER V

FEES

Section 1. Statement of Purpose. These rules and regulations are adopted to implement the board's authority to determine and collect reasonable fees, and to receive and expend funds for the pursuit of the authorized objectives of the board.

Section 2. General Information.

1. (a) The board shall establish the fee schedule not to exceed the statutory limits.
2. (b) The current fee schedule as established by the board shall appear in the official records of the board's activities and shall be kept on file in the board office.
3. (c) At the time of application, a copy of the current fee schedule shall be provided to applicants for all categories of licensure and certification; including licensure and certification by examination, endorsement, relicensure, biennial renewal, reactivation, or reinstatement.
4. (d) Fees shall be payable in the exact amount, by money order, cashier's check, credit card, or cash for all services, and shall be paid in advance of the services rendered. Fees for renewal of licensure may, in addition to money order, cashier's check, credit card, or cash, be paid for by personal check. Refunds will not be made on fee overpayment of $5.00 or less.
5. (e) All fees collected by the board are non refundable.

Section 3. Fees.

1. (a) Fees for services shall be established by the board and shall be based upon or reasonably related to the particular administrative costs involved in each transaction, and a schedule of such fees shall be provided to applicants.

2. (b) Services for which the board charges a fee shall include but not be limited to the following fee schedule:

         2. (II) Licensure by Endorsement for LPNs...........$105.00

      2. (B) Verification of Licensure to All Jurisdictions:

         1. (I) Verification of Licensure................................$ 20.00

      3. (C) Examination Fees (Including a Graduate Temporary Permit):

         1. (I) Examination for RN Licensure......................$100.00
         2. (II) Examination for LPN Licensure....................$100.00
         3. (III) Reexamination for Nurses.............................$100.00
         4. (IV) Examination for CNA Certification..............$ 45.00
