Section 1. Disease List. W.S. 35-4-130 declares Sexually Transmitted Diseases (STD) to be contagious, infectious, communicable and dangerous to public health. STDs fall into two categories: those to be reported within twenty-four hours of diagnosis (category I) and those diseases to be reported within seven days of diagnosis (category II).
Reportable Sexually Transmitted Diseases or conditions that must be reported within twenty-four (24) hours of a diagnosis are:
Chancroid Chlamydia PID Gonorrhea PID Hepatitis B, Acute Syphilis
Reportable Sexually Transmitted Diseases or conditions that must be reported within seven (7) days of diagnosis are:
AIDS HIV Infection Hepatitis B Genital Herpes (HSVII) Lymphogranuloma Inguinale Human Papilloma Virus (Genital Warts)
(a) The following is a list of individuals and facilities which have an independent duty to report the occurrence of sexually transmitted disease:
(i) A physician or other health care provider diagnosing or treating a case of STD;
(ii) The administrator of a health care facility or penal institution in which there is a case of STD;
(iii) The administrator or operator of a laboratory performing a positive test for STD.
(a) The physician must report or cause a report to be made on the official State Disease Case Report, DHMS-PM 032, revised 9/89, or the physician may report via telephone, to be confirmed by using the above form.
(b) The administrator of a health care facility or penal institution must report or cause a report to be made of the diagnosis or treatment of an STD using the official State Disease Case Report, DHMS-PM 032, revised 9/89, or may report via telephone, to be confirmed by using the above form.
(c) The administrator or operator of a laboratory must report or cause a report to be made of test findings for reportable sexually transmitted diseases using the Official NCR Laboratory Report form issued by the Department.
(d) Any physician or other health care provider and any administrator or operator of a health care facility or laboratory or penal institution reporting a diagnosis or positive test result pursuant to W.S. 35-4-130 through 35-4-134 shall notify any health care employee and/or health care professional reasonably expected to be at risk of exposure to a dangerous or life-threatening sexually transmitted disease, e.g., HIV/AIDS, who is involved in the supervision, care, and treatment of an individual infected or reasonably suspected of being infected with a dangerous lifethreatening STD.
(i) Notification shall be verbal.
(ii) Notification shall take place within 24 hours or as soon as possible.
Section 4. Submission of Reports. All STD and HIV/AIDS reports made by the physician or other health care provider, administrator of a hospital, director or supervisor of a laboratory facility, dispensary or penal institution, or any other health care facility shall be forwarded to:

All reports shall be in an envelope marked 'CONFIDENTIAL'.
Section 5. Report Forms. Reporting forms may be obtained from the Wyoming State Department of Health, Preventive Medicine.
