Section 1. Statement of Purpose. These Board Rules are adopted to implement the Board's authority to determine and collect reasonable fees.
(a) Fees shall be payable in the exact amount by money order, cashier's check, or certified check. Application and renewal fees may be paid by personal check.
(b) All fees collected by the Board are non-refundable.
Section 3. Fees. Services for which the Board charges a fee shall include, but is not limited to, the following fee schedule:

| (f) | Return to Practice Fee (lapsed license more than 5 years) | $400.00 |
| (g) | Relicensure Following Discipline Fee | $500.00 |
| (h) | Non-sufficient Funds Fee | $30.00 |
| (j) | Roster Fee | $50.00 |
| (k) | Replacement Wall Certificate | $10.00 |
| (l) | Replacement Pocket ID Cards | $10.00 |
| (m) | License Verification Fee | $15.00 |
| (n) | Copy Fee | $.25 per page |
