CHAPTER 10

FEES

Section 1. Statement of Purpose. These Board Rules are adopted to implement the Board's authority to determine and collect reasonable fees.

Section 2. General Information.

1. (a) Fees shall be payable in the exact amount by credit card, money order, or check.
2. (b) All fees collected by the Board are non-refundable.

Section 3. Fees. Services for which the Board charges a fee shall include, but not be limited to, the following fee schedule:

3. (ii) Application for Licensure by Endorsement $750
4. (iii) Application for Non-Clinical Licensure $50
5. (iv) License Renewal $185
6. (v) Late License Renewal (Jan 1 – Mar 31) $370
7. (vi) Non-Clinical License Renewal $25
8. (vii) Relicensure $370
9. (viii) Reinstatement $750
10. (b) Sedation Permit Holders:
11. (i) Application for Moderate Sedation Permit $500
12. (ii) Application for Deep Sedation/General Anesthesia Permit $500
13. (iii) Application for Operating Dentist Permit $25
14. (iv) Application for Facility Permit $25
15. (v) Office Inspector Fee (paid directly to each inspector) $250
