Section 1. General Information.
(a) The current fee schedule shall appear in the official records of the Board's activities and shall be kept on file in the Board office. (b) At the time of application, a copy of the current fee schedule shall be provided to applicants. (c) Fees shall be payable in the exact amount, by money order or cashier's check, for all services and shall be paid in advance of the services rendered. Renewal fees may also be paid by personal check. (d) All fees collected by the Board are non-refundable.
Section 2. Fees. Services for which the Board charges a fee shall include but is not be limited to the following fee schedule.
(a) Application Fee: Shall not exceed a fee of $500 per applicant. (b) State Examination Fee: Included in Application Fee. (c) Reciprocity Fee: Included in Application Fee. (d) Initial License Fee: Included in Application Fee. (e) Annual License Renewal Fee: Shall not exceed a fee of $500 per year. (f) Renewed Late Fee: June 2-July 1: Shall not exceed a fee of $200. (g) Restoration of Revoked License Fee due to non-renewal: Double the current annual license renewal fee. (h) Replacement Document Fee (pocket card or certificate): $25 (i) Hard-Copy Roster: $100 (j) Verification to Another Jurisdiction: $25
