Section 1. License Types. The Board is authorized to issue one (1) type of license to those applicants who meet the educational, experiential, examination and other qualifications established by the Board and who substantiate those qualifications to the Board's satisfaction.
Section 2. Application Requirements. Every applicant for a license to practice chiropractic in the State shall submit all of the following materials two full weeks prior to examination:
Section 3. When Eligibility is Unclear. When the Board is unable to ascertain from the documents submitted that the applicant meets the above criteria it shall request from the applicant whatever additional documentation or information is necessary to enable the Board to make that decision. The Board may also request the applicant to appear before the Board at a regularly scheduled meeting to clarify issues relating to that individual's qualifications.
Section 4. Issuance of License. Upon verification and acceptance by the Board of the applicant's documentation of qualifications, the applicant shall be granted licensure and the Board shall issue to each successful candidate a certificate bearing the date of issuance and the seal of the Board. The Board shall enclose a letter with each certificate stating the terms of the license and the basis on which such license may be renewed.
