(a) A licensee may prescribe:

1. (1) Topical pharmaceutical agents;
2. (2) oral pharmaceutical agents that are included in the drug formulary established by the board pursuant to section six of this article or new drugs or new drug indications added by a decision of the board; and (3) contact lenses that contain and deliver pharmaceutical agents that have been approved by the Food and Drug Administration as a drug.

(b) An applicant must:

1. (1) Submit a completed application;
2. (2) Pay the appropriate fee;
3. (3) Show proof of current liability insurance coverage;
4. (4) Complete the board required training;
5. (5) Pass an examination; and
6. (6) Complete any other criteria the board may establish by rule.
