1. (1) A certificate of veterinary inspection must meet the requirements in Title 9 C.F.R Part 86 (January 1, 2014) and contain the following information:

   1. (a) An entry permit number, when required;
   2. (b) Date of inspection;
   3. (c) Names and physical addresses of the consignor and consignee;

   4. (d) Shipment information, including:

      1. (i) Physical addresses of origin and destination of shipment;
      2. (ii) Anticipated shipment date;
      3. (iii) Number of animals in the shipment; and
      4. (iv) Purpose for which the animals are to be moved.
   5. (e) Certification that the animals are free from clinical signs or known exposure to any infectious or communicable disease;
   6. (f) Test or vaccination status, when required;

   7. (g) Description of each animal by:

      1. (i) Identifying species;
      2. (ii) Breed;
      3. (iii) Age;
      4. (iv) Sex of the animal;
      5. (v) Color; and
      6. (vi) Official individual identification.

2. (2) Copies of all certificates of veterinary inspection must be forwarded within seven calendar days from date of issuance to:
