1. (1) Any person may request a refund if they can show a specified amount over $50 was paid in error. Refund requests must be received in writing to:

   1. (a) Name and address of applicant;
   2. (b) Total amount of refund request with rationale;
   3. (c) Dated, proof of payment, e.g., check number.
