1. (1) The board shall issue a registration to any applicant who pays any applicable fees and submits, on forms provided by the board, the applicant's name, address, and other information as determined by the board, provided there are no grounds for denial of registration or issuance of a conditional registration under this chapter or chapter 18.130 RCW.
2. (2) Applicants must file an application with the board for registration within three days of employment.
