A monthly disability benefit is not payable for a total disability proximately caused by:
- (1) self-inflicted injury;
- (2) abuse or misuse of any substance, including alcohol;
- (3) illicit drug use;
- (4) an act of aggression committed by the eligible employee; or
- (5) the eligible employee's commission or attempted commission of a felony or other illegal act.
